You can get yourself back to normal weight and remove the negative trait by just eating a bit more heavily for a bit. Same goes for overweight. Lose the weight and lose the negative trait. I take underweight because it still allows me to choose fitness perks if I want. Overweight removes the ability to select them when making your character.
