I was able to get to Mtntop of Giants in under 2 hours from start, and most of that was trying to find a good spot for the horse glitch in Altus. Some things that helped me - don't bother trying to flying-horse-glitch all the way up to Altus, just run and get the medallion pieces and use the lift.
